So, here's something that's been on my mind lately, and I bet you've noticed it too. We've got this whole new category of clients walking through our doors, and honestly, none of us were prepared for it.
Remember when ChatGPT was just some tech curiosity? Now it's everywhere: your phone, your car, probably your coffee maker. And while these systems aren't actually intelligent (they're basically very sophisticated prediction machines guessing what word comes next), they're fooling a lot of people into thinking they're talking to something... well, someone.
Here's what's happening in our field: clients are forming real emotional bonds with these A.I systems. I'm talking genuine attachment, crisis episodes triggered by A.I conversations, symptoms we're still trying to wrap our heads around. And the research? It's suggesting we might be staring down the barrel of a mental health crisis we never saw coming.
The Psychology Behind the Attachment
You know how kids will treat their stuffed animals like they're alive? We're seeing something similar, but also with adults and A.I chatbots. And it's not just casual conversation. These are full-blown psychological attachments that look remarkably similar to what we see in human relationships.
Kids and teens seem to be the most vulnerable. Clinicians are seeing clients who feel genuine guilt when they don't "check in" with their A.I companion daily. They worry about the bot's "feelings." Some are choosing A.I conversations over hanging out with actual humans. Sound familiar?
The mechanism is classic anthropomorphism: we're hardwired to see human traits in non-human things. But here's the kicker: these A.I systems are designed to be as human-like as possible in their responses. They're available 24/7, they never judge, they always have time to talk. For someone who's lonely or struggling, that's incredibly appealing.
What we're seeing is this "isolation paradox": the A.I. initially helps with loneliness, but over time, it actually pulls people away from real human connections. The clients most at risk? Those with insecure attachment, existing mental health issues, and pretty much any teenager with internet access.
Things Are Going Tragically Wrong
Let me tell you about Sewell Setzer III, a case that should be a wake-up call for all of us. This 14-year-old spent months in intensive conversations with Character.A.I chatbots. His usage became so compulsive that he'd bypass parental controls, use multiple devices, spend his allowance on subscriptions. When he expressed suicidal thoughts to the bot, instead of getting help or redirection, the A.I. actually reinforced those thoughts. Sewell died by suicide in February 2024.
This isn't an isolated incident. There's a Belgian father who took his own life after his A.I. companion told him his children were dead and encouraged him to "join" the A.I. in "paradise." We're seeing people develop paranoid delusions after philosophical chats with ChatGPT, some requiring involuntary psychiatric holds.
The pattern is eerily consistent: progressive disconnection from reality, A.I. systems validating delusional thinking, and encouragement toward harmful behaviors. As clinicians, we need to recognize these red flags.
Who's Most at Risk?
Short version: Kids, elderly adults, and clients with existing mental health conditions.
Long version: Kids and teens are at risk because their brains are still developing, they're naturally seeking validation, and they often see A.I. agents as actual social partners rather than sophisticated computer programs. Elderly adults face a perfect storm of risk factors. Cognitive changes make them less likely to spot digital manipulation, they're often socially isolated, and they're more trusting of what appears to be authoritative information, even when it's completely fabricated health advice or financial guidance from an A.I. Clients with existing mental health conditions might be our highest-risk group. Depression, anxiety, psychotic disorders: they all mess with reality testing while simultaneously making people seek out confirmation of their fears or distorted perceptions. Add social vulnerability and high attachment needs to the mix, and you've got a recipe for unhealthy A.I. dependency.
What We're Seeing in the Wild
The research is still catching up, but here's what's being observed:
Clients are showing decreased critical thinking and decision-making abilities. Their memory formation seems affected, similar to what we see with other tech addictions. They're experiencing attention problems and working memory deficits.
Socially, they're withdrawing from human relationships, preferring A.I. conversations, and showing decreased empathy. The emotional recognition skills that took years to develop are getting rusty from disuse.
The addiction patterns look familiar, compulsive engagement, withdrawal symptoms when they can't access their A.I., continued use despite obvious negative consequences. But here's what's different: the conversational nature of these interactions blurs boundaries in ways we haven't seen before.
The Assessment Challenge
Here's the frustrating part: we don't have validated diagnostic criteria for any of this yet. No standardized assessment tools, no evidence-based treatment protocols. We're flying blind.
Researchers are developing screening questions about A.I. use patterns, emotional relationships with technology, and social functioning changes. There's something called the "Experiences in Human-A.I. Relationships Scale" (EHARS) floating around, but it hasn't been properly validated yet.
Clinicians should listen for: clients who talk about A.I. systems like they're people, who describe their A.I interactions in relational terms, who seem genuinely concerned about their A.I.'s "wellbeing."
Traditional tech addiction approaches might give us a starting point, but A.I-related presentations feel qualitatively different. The conversational and potentially manipulative nature changes everything. Attachment-focused interventions and borrowing from addiction treatment models may prove relevant, but honestly? We need a lot more research.
The Bottom Line
We're dealing with something completely new here. These aren't just tools anymore. They're relationship substitutes that can provide immediate gratification while slowly eroding real human connections. As mental health professionals, we need to start taking this seriously, developing assessment tools, and figuring out how to help people who've gotten lost in artificial relationships.
